In many less developed nations, though, the virus is raging on, sometimes out of control, while vaccinations are happening far too slowly to protect even the most vulnerable.<\/p>\n<p class=\"css-axufdj evys1bk0\">That split screen \u2014 clubs and restaurants reopening in the United States <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.bbc.co.uk\/news\/entertainment-arts-56962231\" target=\"_blank\" rel=\"noopener noreferrer\">and Europe<\/a> while people <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/05\/03\/world\/asia\/India-coronavirus-deaths-oxygen.html\">gasp for oxygen in India<\/a> \u2014 was never supposed to be so stark. Some 192 countries signed up last year for <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2020\/12\/18\/world\/covid-covax-vaccine-deals.html\">Covax, a vaccine-sharing partnership,<\/a> and the Gates Foundation poured $300 million into an Indian factory to make doses for the world\u2019s poor. The European Union\u2019s top executive <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/ec.europa.eu\/commission\/presscorner\/detail\/en\/speech_20_996\" target=\"_blank\" rel=\"noopener noreferrer\">told a global summit<\/a> last June: \u201cVaccination is a universal human right.\u201d<\/p>\n<p class=\"css-axufdj evys1bk0\">But the virus is <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/interactive\/2021\/05\/01\/world\/virus-cases-world-peak.html\">spreading more rapidly than ever<\/a>, driven largely by gains in South America and India, and the campaign to vaccinate the world is floundering.<\/p>\n<p class=\"css-axufdj evys1bk0\">India, an important source of vaccines in normal times, has halted exports as it fights <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/04\/24\/world\/asia\/india-coronavirus-deaths.html\">a record surge in the virus<\/a> and an expanding humanitarian crisis. That has delayed critical shipments, with India making the majority of Covax supplies.<\/p>\n<\/div>\n<\/div>\n<div id=\"story-ad-1-wrapper\" class=\"css-1r07izm\"><\/div>\n<div class=\"css-1fanzo5 StoryBodyCompanionColumn\">\n<div class=\"css-53u6y8\">\n<p class=\"css-axufdj evys1bk0\">In Brazil, where <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/03\/27\/world\/americas\/virus-brazil-bolsonaro.html\">thousands are dying daily<\/a>, officials have received only a 10th of the AstraZeneca doses they were promised by midyear.<\/p>\n<p class=\"css-axufdj evys1bk0\">And in countries as varied as Ghana and Bangladesh, which blew through their initial vaccine supplies, the lucky few who received a first shot have been unsure of when they will receive another.<\/p>\n<p class=\"css-axufdj evys1bk0\">\u201cIt\u2019s a moral issue,\u201d said Boston Zimba, a doctor and vaccine expert in Malawi, which has vaccinated only 2 percent of its people. \u201cThis is something rich countries should be thinking about. It\u2019s their conscience. In the absence of enough funding for chronically underpaid health workers and vaccination training, many of the few doses that have been delivered are sitting in warehouses, with expiration dates rapidly approaching.<\/p>\n<p class=\"css-axufdj evys1bk0\">The stuttering global rollout is having calamitous consequences. Unvaccinated nations are being walloped by the virus. <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/05\/02\/world\/india-covid-variants.html\">New variants could emerge<\/a> in reservoirs of untamed infections, prolonging the pandemic for rich and poor nations alike. The global economy stands to suffer <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/01\/23\/business\/coronavirus-vaccines-global-economy.html\">trillions of dollars in losses<\/a>.<\/p>\n<p class=\"css-axufdj evys1bk0\">Yet, for all the determination of global leaders to right past wrongs \u2014 most recently, a scarcity of swine flu vaccines in poorer nations during <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/topic\/subject\/swine-flu-h1n1-virus\">that 2009 pandemic<\/a> \u2014 the difficulties facing the current inoculation campaign are steep.<\/p>\n<p class=\"css-axufdj evys1bk0\">When the pandemic exploded into view last year, Covax was short on funds, making it impossible to compete with richer nations in locking down contracts for vaccines. More recently, the United States, the European Union and India have barred at least some vaccine-related exports, leaving regions that do not make their own doses dependent on those that do.<\/p>\n<p class=\"css-axufdj evys1bk0\">Lately, Western nations have begun promising vaccines to the developing world \u2014 60 million AstraZeneca <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/04\/26\/us\/politics\/biden-vaccine-india.html\">doses from the United States<\/a>, one million AstraZeneca <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.cnbc.com\/2021\/05\/03\/covax-gets-a-boost-from-moderna-deal-swedens-donation-of-covid-vaccines.html\" target=\"_blank\" rel=\"noopener noreferrer\">doses from Sweden<\/a>. But these \u201cdonations\u201d are a drop in the bucket, and have in some cases been haphazardly planned, leaving countries too little time to administer the doses.<\/p>\n<p class=\"css-axufdj evys1bk0\">Even as richer nations inoculate their own citizens, they may start saving vaccine-making capacity for booster shots to deploy against new variants, another blow to countries bereft of manufacturing bases.<\/p>\n<div class=\"css-1fanzo5 StoryBodyCompanionColumn\">\n<div class=\"css-53u6y8\">\n<p class=\"css-axufdj evys1bk0\">Vaccine makers are prospering off sales to the world\u2019s rich. Pfizer <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/05\/04\/business\/pfizer-covid-vaccine-profits.html\">brought in $3.5 billion from its vaccine<\/a> in the first three months of 2021, while it has promised Covax less than 2 percent of this year\u2019s doses. There is also the <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/05\/03\/world\/global-coronavirus-vaccine-shortage.html\">sheer difficulty of making so many doses<\/a>.<\/p>\n<p class=\"css-axufdj evys1bk0\">Pressure has been building on the Biden administration <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/05\/03\/us\/politics\/biden-coronavirus-vaccine-patents.html?action=click&amp;module=Top%20Stories&amp;pgtype=Homepage\">to suspend intellectual property rights<\/a> on vaccines to encourage wider manufacturing. But given the production difficulties, that step could take years to yield results, experts say.<\/p>\n<p class=\"css-axufdj evys1bk0\">However much it costs, and however long it takes, Western nations should also pressure vaccine makers to partner with global manufacturing sites, analysts said. Ramping up production in that way can be bumpy, but surveys last year suggested there was untapped capacity.<\/p>\n<p class=\"css-axufdj evys1bk0\">And local manufacturing can temper fears of foreign-made vaccines. \u201cThe end game is to bring vaccines that are built and manufactured in Africa,\u201d said President F\u00e9lix Tshisekedi of the Democratic Republic of Congo, \u201cbecause it will create trust and confidence in people because they know it\u2019s a vaccine made by us and for us.\u201d<\/p>\n<p class=\"css-axufdj evys1bk0\">For Covax, a heavy reliance on shipments from India has been costly. In January, it forecast having 235 million doses by April and 325 million by May on its way to two billion doses this year, enough to vaccinate 20 percent of people in recipient countries, according to planning documents saved by Zain Rizvi, a medicines access expert at Public Citizen, an advocacy group.<\/p>\n<p class=\"css-axufdj evys1bk0\">But by March, that forecast had been slashed by roughly a third. And as of Tuesday, Covax had shipped 54 million doses, less than a quarter of its earlier April target. Since vaccinations began in April, only 1,888 people have been inoculated, forcing the country to send most of its supply to nearby nations to save it from expiring.<\/p>\n<p class=\"css-axufdj evys1bk0\">Ivory Coast received 504,000 AstraZeneca doses from Covax in late February, yet to date has administered 155,000. At vaccination sites in Abidjan, the country\u2019s largest city, waiting chairs sit empty and health workers play on their phones to pass the time.<\/p>\n<p class=\"css-axufdj evys1bk0\">Western warnings about very rare vaccine side effects have spread on social media, said Dr. Ars\u00e8ne Adepo, making people wonder why a shot that much of Europe had temporarily rejected was fit for Ivorians. Those worries compounded fears that Africans were being used as \u201cguinea pigs\u201d and that the shots would reduce fertility.<\/p>\n<p class=\"css-axufdj evys1bk0\">Yet vaccine confidence collapsed in part because the government did so little to build faith in the first place: Ivory Coast health officials said that a public awareness campaign would begin only this week, more than two months after the first doses arrived. Of the 92 poorer nations being supplied by Covax, eight have cut their health budgets because of virus-related economic losses, and several others are struggling to fund their health systems in part because they do not qualify for grants or more generous loans, Mr. Schreiber said.<\/p>\n<p class=\"css-axufdj evys1bk0\">The World Bank has promised $12 billion for vaccine rollouts, but has so far approved $2 billion in projects. In mid-March, the bank found that less than a third of low- and middle-income nations reported having plans to train enough vaccinators or campaigns in place to fight vaccine hesitancy, said Mamta Murthi, the bank\u2019s vice president for human development.<\/p>\n<p class=\"css-axufdj evys1bk0\">Many countries\u2019 needs are even simpler. Some cannot pay to print immunization cards. Malawi, which is <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/www.nytimes.com\/2021\/04\/16\/world\/vaccine-hesitancy-africa.html\">planning to destroy 16,000 doses<\/a> that arrived shortly before they expired, has struggled to cover lunch allowances for health workers traveling from one facility to another to give vaccines.<\/p>\n<p class=\"css-axufdj evys1bk0\">The outlook is uncertain. More doses will breed more vaccine confidence, said Freddy Nkosi, the country director in the Democratic Republic of Congo for VillageReach, a health nonprofit. But if India\u2019s outbreak keeps surging, the chief executive of the Serum Institute of India <a class=\"css-1g7m0tk\" title=\"\" href=\"https:\/\/apnews.com\/article\/world-news-india-global-trade-coronavirus-pandemic-dd58b5c596f261f8a5655e8d8b5d6984\" target=\"_blank\" rel=\"noopener noreferrer\">said recently<\/a>, \u201cWe are going to have to keep supplying to India, and not anywhere else.\u201d<\/p>\n<\/div>\n<\/div>\n<div id=\"story-ad-6-wrapper\" class=\"css-1r07izm\">\n<div class=\"css-1fanzo5 StoryBodyCompanionColumn\">\n<div class=\"css-53u6y8\">\n<p class=\"css-axufdj evys1bk0\">Ruth Bechtel, VillageReach\u2019s country director in Mozambique, said that expanding the vaccination drive as more doses arrived would create new logistical hurdles. The country has administered roughly a quarter of the doses it has received.
